Since Daoc release until patch 1.75 you got a target as soon as you pressed down your mouse button (with cursor over a target). After 1.75 there was a bug or "feature" that would not give a target until you pressed and released the mouse button.
1. Put cursor over a target
2. Press down left mouse button
3. Release left mouse button
Fpr true 1.65 change it to receiving a target when pressing down the mouse button or atleast add an option so players can decide how the mouse/targeting should behave.
