I have spent much time with a couple of toons in Cursed Tooms and killed Draugr Commander many times. He has never droped "Draugr Commander Axe". I have seen him drop "Ghostly Gem" and "Rusty Draugr Commander Medal".
1. Take a character capable of farming a lvl 25/26 mob.
2. Go to Cursed Tomb and go to the room loc. 27219,24299
3. Kill the 'draugr commander'. He never drops 'draugr commander axe'
Add the 'draugr commander axe' to the 'draugr commander's loot table.
