1. Do lvl 40 shadowblade epic quest (The War Continues)
2. choose 1 hand sword upon completion

The quest should reward an item of choice. Worked for a friend who chose axe, failed for myself with the sword option.
No quest reward in inventory and the quest was completed within the journal.
