The epic quest lvl 20 forces you to kill Fressen. So kill him and the next step should start. I killed him twice without success / quest update.
1. Get lvl 20 healer/shaman epic quest.
2. Kill Fressen.
3. Get next step by killing Fressen several times.

If you killed Fressen once - the quest should be updated.
I'm pretty sure, that then task is to kill him once. Twice or more would be possible, but lets just say its a bug.
