I fell through the floor in Camelot at loc 25916,18223 - this can be reproduced.
1. got to loc 25916,18223
2. fall through the floor
3. try again and fall again :)
I think it is obvious that the char should not fall through the floor.
The character should not drop through the floor (unintended behavior).
