4x99 on 5 OC and BOOM!!!

Yesterday was trying to imbue an item with 4x99 qua gems 5 OC and ................ boom;p

It never happend to me before so i am little confused;p (Imbued like 1000s of items) so question is any1 have had the same situation?:P
I know it is 1% of BOOMing but as i said it never happend to me before;>

It's 1% of failures
So yes it's rare but it happens

If you want to prevent boom do 3 x 99 and 1 MP and 0% of boom at max OC (5,5) :!:
