A friend of mine and I are looking to lvl a couple toons together. We're looking at a combo of BD or SM and Sham or Healer. Being viable in RvR when we get done would be nice. It's been years since we played last, and we never played mid before. We have no plans on buffbotting, dualboxing or the like. What would be a fun, effective combo for leveling, for both players?

pac healer and summon sm level faster than any other duo and sm respec dark for thidranki for rvr would be fun and fast.

I d say Bard+Eld, woops wrong realm
Paci+SM is quiet good in PvE but keep in mind that a SM is not that welcome in RvR groups, so I d go Aug Healer+Dark Runi and build tank grps on ur own.

Dont roll a SM if you wanna do some RvR in the end.
Runemaster + Healer is quite a good duo, BD + Healer might be even better in most situations.
But RM is better for 8vs8 and hence its alot easier to find a rvr grp - I would go for RM + Healer.

Can't speak for healer/caster, but my wife and I have been doing very well as a war/clr combo. Our only real issues have been our gear getting below our level. After some helpful advice fromt he forums we realized that we had to hunt in some dungeons now and then for the drops. We had just being doing outdoor zones and killing anything that conned red and below for the camp bonuses. Once we mixed the dungeons in we caught up on drops a little. We've hit lvl 25 pretty easily as this combo. I don't know what the later elvels will be like, but we still seem to be going pretty strong.

Be sure to hit DF as much as possible. You can start gathering DF seals now. You can always buy DF armor. It's not crafted but it's good to level with. Also be sure to go to West Skona at 33 and find the named mob down on the south side of the southwestern most lake. She's named Askashoa or something. Looks like a hib elf maiden. Kill her and you get a great vest and legs. Farm her twice for both you. You'll need to let your wife get the kill one time I believe.
