For what I know, shield size increase the number of enemies from you can block attacks. Little 1, medium 2, large 3. When I fight with 3 enemies and I use a medium shield, should I block attacks from 2 of them or it's like I don't have a shield and I don't block attacks from anyone?

Fighting 3 opponents with a medium shield will grant you a chance to block against two of them - the third one will fight you as if you had no shield.

Thanks, is there any special order in which i choose the 2 enemies to block? Like the first 2 who attacks me? or it's random every swing

It's random. You have X chances to protect yourself with your shield from X opponents (depending on shield size).
